SCHOLARSHIP APPLICATION

STUDENT INSTRUCTIONS

Complete the application
questionnaire
. The questionnaire allows you to acquire the information and documentation you will need to complete your application.

Complete the scholarship
application
. Be sure to include the following items with your submission.

  1. Completed scholarship application including short
    essay
    .
  2. Letter of recommendation from a current High School teacher.
  3. Letter of recommendation from a current officer, committee member or advisor of the Filipino-American Community of Southern Colorado.
  4. Copy of last grade report showing current GPA.
  5. Copy of ACT score report.
  6. Copy of acceptance letter from your college of choice.

The deadline for applications to be received by the Scholarship Committee is May 31, 2004. There will be no extensions to the deadline. Any application received after May 31, 2004 will not be reviewed.
