  4. Do you or any of your immediate family members participate in the activities of the Filipino-American Community of Southern Colorado? Yes No
  5. Do you have a letter of recommendation from at least one of your current high school teachers? Yes No
  6. Do you have a letter of recommendation from at least one of the current officers, committee members or advisors of the Filipino-American Community of Southern Colorado? Yes No
